PowerScribe 360 end of life: what radiologists need to know

The dates, plainly: Microsoft notified customers in February 2026 that PowerScribe 360 is being retired. Maintenance ends August 31, 2026. Full end of life is August 31, 2027 (The Imaging Wire, quoting Microsoft).

What the two dates actually mean

After August 2026, PS360 stops receiving patches and updates. It will keep running, but anything that breaks (an OS update, an interface change, a RIS/PACS upgrade on the other end) stays broken. After August 2027, support ends entirely. Most groups will be forced onto a migration timeline whether or not the product still works day to day.

Where everyone is being moved

The destination is PowerScribe One, which is subscription-licensed and hosted in Microsoft's cloud. As of June 2026, Microsoft reported 250+ organizations and 10,000+ radiologists on PS One. Set against the roughly 80% of US radiology reporting that runs on PowerScribe, that means the majority of PowerScribe seats are still on PS360 or mid-migration right now. If your group has not scheduled its move yet, it is not behind; it is typical.

The licensing change is the part that generated pushback: capabilities groups already owned under perpetual PS360 licenses come back as recurring subscription fees, and AI features are not automatically included (Smart Impression, for example, is a separately licensed add-on).

What migration means for you personally

Your voice profile does not simply come along. Expect re-enrollment or retraining on the new platform, and expect recognition to be rough while the new profile learns you. Plan for it during a lighter stretch if you have any say in timing.
AutoText and templates need verification. Migration tooling exists, but field behavior, picklists, and formatting are worth checking template by template before your first full shift on PS One.
The timeline is not yours. Contract cycles, RIS/PACS integration work, and IT staffing decide when your group moves. The dates above are the outer boundary, not the plan.

Questions worth asking your group now

When is our migration actually scheduled, and what happens if it slips past August 2027?
What happens to my custom vocabulary and AutoText, and who verifies them after the move?
Which AI features are in our PS One contract, and which are separately licensed per radiologist?
What is the rollback plan if recognition quality drops after migration?

If your group rides PS360 to the end

Plenty will. The software will keep working after maintenance ends, but on a frozen engine with no fixes coming. The day-to-day pains (profile drift, recognition inconsistency, crashes that IT can no longer escalate) tend to grow through exactly the window when the group is busiest planning the migration.
